carbapenems Flashcards
Carbapenems are synthetic β-lactam antibiotics that differ in structure
from the penicillins - how?
sulfur of thiazolidine ring is externalized and replaced with Carbon
drugs in this class
imipenem
meropenem
ertapenem
doripenem
why is imipenem used with cilastatin
Imipenem is compounded with cilastatin to protect it from metabolism
by renal dehydropeptidase
imipenem resists hydrolysis by most β-lactamases, but not the
Metallo beta lactamase
route of adm of carbapenems
IV
excretion?
renally(glomerular filteration)
what are the broadest spectrum Beta lactams
imipenem/cilastatin and meropenem
why is imipenem alone harmful?
Imipenem undergoes cleavage by a dehydropeptidase found
in the brush border of the proximal renal tubule. This enzyme
forms an inactive metabolite that is potentially nephrotoxic.
uses of carbapenems?
*Used as empiric therapy.
*Active# penicillinase producing G+ve, G-ve, anaerobes & p. aeruginosa.
adrs of carbapenems
Imipenem/Cilastatin→ N, V, and diarrhea.
High doses→ seizures. (Not seen with Meropenem).
