Definition of bursitis
Inflammation of bursa
What is bursa
Small sack lined with synovium, reduces friction between tendon/bone
Can you palpate bursa?
Not palpable unless inflamed
Causes of bursitis
-Overuse of structure surrounding the bursa
-Usually secondary condition
~to other conditions like tendinitis
Contributing factors of bursitis
-Muscle imbalances
-Poor Biomechanics
-Postural dysfunctions
Common bursitis locations
-Shoulder bursa
>subacromial+subscapular
-Olecranon bursa
-Trochantric bursa
-Iliopectineal bursa
-Ischial bursa
-Knee bursae
>pes anserine
>infrapatellar
>prepatellar
-Retrocalcaneal bursa
Medical treatment bursitis
-Rest
-NSAIDS
-Ultrasound
-Ice
-Can protect superficial bursae with padding- donut pillow
-Aspirate- drawing fluid out with needle, filled with corticosteroid
Surgery
-Stretch + strengthening structures that cross bursa
Other bursal injuries + where they are
-Bakers cyst- lat side popliteal
-Bunion- at first metatarsophalangeal joint capsul
Baker cyst
-Not overuse
-In children + adults
-Left untreated usually resolves itself
-Local contraindication (avoid area)
Bunion
-Excessive bone growth- callus + inflammation
-Thickening bursa over joint
-Result to joint hypermobility + poor Biomechanics
-Local contraindicated (avoid area)
Contraindications bursitis
-Acute- avoid pressing bursa, causing drag to surrounding structures
-If infected- medically treated
Symptom picture bursitis acute stage
-Bursa irritated by surrounding structures
-Inflammation, heat, swelling
-Px is deep + burning during rest/activity
-Pain may refer from bursa
-May affect sleep
-ROM affected, joint restricted
-Other conditions may be present (tendinitis)
Symptom picture bursitis chronic stage
-Pain/achiness with activities or direct compression
-Chronic inflammation, fibrosis + adhesions
-ROM less affected
