Bullous pemphigoid-Eti
Bullous autoimmune disorder in elderly- 60 onset, men more frequently.
Bullous pemphigoid- Sx
Prodrome followed by bullae in a generalized eruption. Moderate to severe pruritus. Erythematous, small, tense lesion in flexural areas- containing serous or hemorrhagic fluid.
Bullous pemphigoid- Dx
Antibody test or biopsy
Bullous pemphigoid- Tx
Ultra potent topical corticosteroids. Systemic predisone if severe
Pemphigus vulgaris- Eti
Autoimmune disorder- rare, most common in jewish or Mediterranean descent. Onset 40-60 YO
Pemphigus vulgaris- Sx
Lesions begin on oral mucous membranes. Flaccid bullae turn to crusts and erosions. Painful.
Pemphigus vulgaris- Dx
IgG antibodies in serum. Light microscopy. Niklosky sign- rubbing swab on uninvolved skin causes separation
Pemphigus vulgaris- Tx
Glucocorticoids & immunosuppresive therapy. Agressive tx required
