3
Q

fetal growth

A

first half - 10% full term weight - hard to recognize
second half - rapid increase
last three weeks - slowed growth - prepare itself before it gets expelled

Q

four stages of labour

A
  1. dilation and effacement
  2. baby emerges

9
Q

Dilation and effacement

A

2-3 weeks with early contrations before called LATENT LABOUR
ACTIVE labour has true dilation - water breaks
pressure against uterus - cervix causes it to dilate - enlarge and efface - thin out
affected ROM, woman can feel it
mucal plug - cervical opening

Q

atypical presentations

A

brow
face
shoulder
breech - legs straight up or hugged - frank breech, butt down
- complete breech, bent knees
-footling breech, feet first, head last - long time in birthing canal which is bad

16
Q

birth difficulties 2

A

precipitous delivery

prolonged labour

17
Q

precipitous delivery

A

less than 3 hours
not dialated enough, cervix and uterine lining not prepared, medical situation/environment
skull bones rapidly compressed - unattended birth

18
Q

prolonged labour

A

only refers to active labour
more than 24 hours
increased chances of infection, suffocation and decreased O2
the baby is in the birthing canal and has left aqueous environment - now theres a delay into the airboure because you need to cut the umbilical cord.

19
Q

adjustments at birth

A

cardiovascular system interaction with the respiratory system which was used minimally as they relied on the mother’s system through the umbilical cord

20
Q

internal respiration age of viability

A

week 25 0 properly function

21
Q

fetal development of CVS heart

A

week 3 - 2 endocardial tubes that fuse into the primitve heart tube
week 5 - it folds, twists and partitions into 4 chambers and development of blood vessles
week 9 - blood vessels establish

22
Q

congenital heart disease

A
born with it 1/5000
holes between chambers
incorrect number of valves
complete absence of chambers
can be detected and treated
23
Q

fetal development of lungs

A

4 weeks - single pouch
16th week - bronchiole tree
3rd trimester - alveoli begin - not fully developed till after we are born

24
Q

fetal lungs are protected by 3

A

foramen ovale, ductus arteriosus and ductus venosus

25
Q

why do fetal lungs need protection?

A

too fragile to handle forces

26
Q

fetal blood gets sent to

A

right heart but mostly oxygenated anyway, shunted by left and right atrium as instead of sending all blood to RV, some brought to left atrium so not everything goes to the lungs

27
Q

ductus arteriosus

A

opening into aorta from right ventricle to lungs, reduce the amount of blood going to the lungs - closes within 30 mins of being born

28
Q

ductus venosus

A

all blood wants to go to the liver but DV shunts away good blood from liver and sends it to systems that need it

29
Q

At birth, without the umbilical cord

A

increased aortic pressure
lungs expand - connection of inhalation with cardiovascular system
ductus venosus doesnt close right away, may last up to a week
