what kind of patient gets systemic fungal infections
super sick ones –> usually immunosuppressed
4 classes of antifungals
polyene antibiotics
azoles
echinocandins
pyrimidine analogs
Amphotericin B
drug of choice for most fungal infections
super toxic (kidneys, low potassium, bone marrow)
Amphotericin B administration
Only IV, but tends to cause phlebitis, so picc or central line best
give with crystalloid to prevent renal issues
avoid other nephrotoxic drugs
azoles
broad spectrum alternatives to Amphotericin B
less toxic and can be given PO, but reacts with a bunch of dif meds
Fluconazole
fungistatic
good for yeast infections –> just one dose
Voriconazole
candidemia
invasive aspergillosis
esophageal candidiasis
Echinocandins
newest class of antifungals
disrupt fungal cell wall
Micafungin is main one
IV only
Superficial mycoses
caused by candida and ringworm infections
treated with candidiasis
Nystatin is topical
