Anatomy (Breast & Thoracic Wall) Flashcards
Retromammary Space
- allows breast to move against pectoralis major
Mammary Gland Innervation
- Intercostal nerves 4-6 (ventral primary rami)
- Contain somatomotor, somatosensory, & autonomic fibers
Breast Innervation
- Sensory fibers
- Sympathetic fibers (blood vessels and smooth muscle in skin and nipple)
Lymphatic Drainage of Breast
- Subareolar lymphatic plexus then..
- Axillary nodes (most), pectoral (anterior), subscapular (posterior), lateral, central, apical then..
- Parasternal nodes then..
- Abdominal nodes
Malignancy in Breast (5)
- derived from epithelial cells of lactiferous ducts
- usually spreads through lymphatics, but venous system is worse
- inference of lymph drainage produces edema of skin (orange peel)
- can thicken suspensory ligaments (fingertip sized dimples)
- can fix breast to thoracic wall
Endothoracic Fascia
- separates parietal pleura from thoracic wall
Costal Groove
- “VAN” from superior to inferior
- Vein
- Artery
- Nerve
- Located below each rib
Needle Location for Thoracocentesis
- superior to a rib so you don’t hit “VAN”
Vein that Drains Brain
- Internal jugular veins (left & right)
Phrenic Nerve
- Innervates diaphragm
- C3, C4, C5
Fibers in Diaphragm
Somatomotor - skeletal muscle
Somatosensory
Postganglionic Sympathetic - blood vessles
Diaphragm Contraction
- Inspiration
- Lowers central tendon
Vena Cava Foramen Contents
- Inferior vena cava
Esophageal Hiatus Contents
- Esophagus
- Vagus Nerve
Aortic Hiatus Contents
- Aorta (then is called abdominal aorta)
- Azygos vein
- Thoracic duct
Thoracic Cavity Expansion During Inspiration (3)
Superior/Inferior - action of diaphragm
Anterior/Posterior - ribs pump handle movement
Lateral - ribs bucket handle movement
Quite Inspiration Muscles
- Diaphragm
Quiet Expiration Muscles
- Passive recoil
Forced Inspiration Muscles
- Diaphragm
- External inercostals
- Scalenes
- Sternocleidomastoid
Forced Expiration Muscles
- Abdominal wall muscles
- Internal intercostals
- Transversus thoracis
Intervertebral Joints
- Cartilaginous
- Intervertebral discs
Costochondral Joints
- Cartilaginous
- Between rib and cartilage
First Sternocostal Joint
- Cartilaginous
- Between first rib and manubrium
Manubriosternal Joints
- Cartilaginous
- Between parts of sternum
Xiphisternal Joint
- Cartilaginous
- Between xiphoid process and sternum
Costoverterbral Joints
- Synovial
- Between head of rib and vertebrae OR
- Between transverse process and costal tubercle
Second - Seventh Sternocostal Joints
- Synovial
- Betweeb ribs 2-7 and sternum
Interchondral Joints
- Synovial
- Between adjacent cartalige on ribs not connected to sternum
