Adult Patient With Lump In Groin Flashcards
Commonest hernia?
Ingual Hernia
2 signs of an ingual hernia
Cough impulse, reduce when lying down and appear while standing
A negative ring occlusion test means?
Indirect Hernia
A positive ring occlusion test means?
Direct Hernia
Explain the ring occlusion test / Deep ring test
Patient is asked to reduce the hernia, then Dr palpates, and asks patient to cough
Which hernia type can descend into the scrotum?
Indirect hernia.
A complication of hernias, is Incarceration, what is that?
A hernia which simply can’t be reduced back into the stomach. Irreducible.
Another complication of hernias, could be intestinal obstruction, what are signs of that
Abdomen pain and distention, vomiting, constipation.
What is the major life threatening complication of abdominal hernias?
Strangulation.
Signs of strangulation
Tense hernia, extremely tender, irreducible. No impulse on cough.
What is Richter’s Hernia?
A small strangulation that doesn’t cause intestinal obstruction
Explain the Bassini repair of ingual hernia
The sac is removed from the cord, opened, and any contents emptied.
Explain the Lichtenstein tension-free repair
The hernia is removed, and a mesh is inserted. A few sutures hold the mesh to the ingual ligament. It’s the Most Common repair for Ingual Hernias.
Advantages of Lichtenstein tension-free repair?
Low recurrence rates, quicker post op recovery.
What treatment do we pursue for femoral hernias of a high risk of bowel strangulation?
McEVEDY.
