Log Book
The incident report forms and a copy of this Policy will be kept in the vehicle log book.
Arrange for….
….other Emergency Services to attend and if necessary render First Aid.
Protect the scene and persons involved…
with warning devices. (four way flashers or traffic cones)
Notify Police
of all incidents.
Notify their DivC or DC
The driver or the officer of a fire department vehicle involved in an accident shall immediately notify their DivC or DC.
If the damage is excess of ________ the on-duty Chief will be notified.
$2500.00
If personal injury results from the accident….
….the on-duty Chief and the Health and Safety Committee will be notified.
If the accent is serious or if Mechanical fault is suspected notify….
-the DivC of Maintenance, as well as Reps of the Health and Safety Committee.
Admission of guilt.
Do not make ant admission of guilt regarding the City of Brampton’s actions. Let the claimants do the talking. Do not volunteer information.
Incident Reports
All vehicles will carry a supply of Incident Reports which are to be completely filled out including the police occurrence number. These forms are to be forwarded to Administration immediately. Administration will notify the Insurance Division. Photographs should be taken if possible.
Based on the accident details and to ensure the safety of the employees as well as of the public, a determination will be made by….
….the on-duty Chief as to whether or not the driver should be immediately removed from driving.
The Accident review Committee consists of?
- Fire Chief or designate
- Department Driving Coordinator
- Shift Driving Instructor(where applicable)
- DivC of Maintenance
- Labour H&S Committee Rep
- If applicable the captain
The purpose of the Accident review Committee
- will meet to determine if the accident was non-preventable or preventable
- Th driver of the apparatus will also be required to attend a portion of this meeting . depending upon the complexity/seriousness of the accident, the risk $ insurance manager may be involved at the committee level.
If it is determined that the accident was preventable the Fire Chief or designate will…
…determine if disciplinary measure are required and what course of action should be taken based on the committee’s findings. This could involve further training, testing, revising response procedures and/or suspension from driving department vehicles until the Department Driving Coordinator is satisfied that the driver is capable of driving fire department vehicles safely.
The driver does not agree with the decision made by the committee….
The driver of the apparatus can ask for an appeal if they do not agree with the decision of the committee. In this particular situation a third party will b brought in such as the Ontario Safety League whose decision will be final after reviewing the circumstances surrounding the accident.
