Lost or Stolen Fuel Cards

If both fuel cards are missing or lost

A fuel card from another apparatus may be used to fuel the vehicle upon approval from the DivC or his designate. The fuel fuel receipt must specify which apparatus was fuelled.

Monthly Maintenance Report

shall be submitted for each vehicle at the end of each month.

All fuel receipts for previous month will be attached to the left hand corner of the Monthly Maintenance report and sent to the A&M division the first week of every month through interoffice mail.

If the reports are not received by the second week of the month…
