what is PR?
inflammatory condition of elderly
what condition is closely linked to PR?
Giant cell arteritis
who is affected?
F>M (2:1), rarely under 50 yrs - usually 70+ years
how does PR present?
old woman with sudden onset of shoulder +/- pelvic girdle stiffness
ESR result?
usually >45 (often ~100)
which condition of the blood do patients of PR often suffer?
anaemia
name 4 systemic symptoms associated?
malaise, weight loss, fever, depression
rheumatology symptoms?
arthralgia/ synovitis
what information is needed to make diagnosis? (3)
age >50, ESR >50, dramatic response to steroids
name 4 differentials?
myalgic onset inflammatory joint disease, malignancy, hypo/hyper-thyroidism, fibromyalgia
name 2 malignancies which are commonly associated with PR?
multiple myeloma, lung cancers
management?
prednisolone 15mg daily for 18-24 month course
prophylaxis?
bone prophylaxis
