Injury and Illness Prevention Program (IIPP)
The PRIMARY OBJECTIVE of this IIPP is to maintain a safe and healthy work environment for all LAFD employees.
The IIPP pursues this objective through the following 9 elements:
- SAFETY RESPONSIBILITIES
- SAFETY COMMUNICATION
- EMPLOYEE COMPLIANCE WITH SAFE WORK PRACTICES
- HAZARD ASSESSMENT AND INSPECTION
- ACCIDENT/EXPOSURE INVESTIGATION/NEAR MISS INVESTIGATION

- TRAINING AND INSTRUCTION
- RECORD KEEPING
- WORKPLACE VIOLENCE
IIPP — Safety Responsibilities of Station/Company Commanders:
-Enforce the safety responsibilities of employees and hold them accountable for their performance.
-Respond to notice of a hazard or unsafe work practice and take the necessary steps to eliminate the hazard.
-Maintain safety training records for employees.
-Maintain access to a current list of hazardous chemicals and the respective Safety Data Sheets (SDS) for which employees may be exposed.
This Injury and Illness Prevention Program is intended to provide guidance for complying with the safe work principles identified in Title _____, California Code of Regulations (CCR), Section 3203 and for minimizing employee exposure to safety and health risks at all LAFD facilities and worksites.
Title 8
Hazard Assessments and fire facility inspections are to be conducted when one or more of the following conditions occur:
-When the IIPP is established.
-When new equipment creates an unsafe condition.
-When a product, process, or procedure creates a hazard or unsafe condition.
-When a new unrecognized hazard or unsafe condition is identified.
-When an occupational injury or illness occurs.
-When a workplace condition warrants an inspection.
-Facility inspection can be conducted during scheduled Battalion/Annual inspection.
Hazard Assessment and Inspection — All Employees
Inspect tools, equipment, machinery, safety equipment, and worksite and personal protective equipment prior to use to identify any items that pose a potential safety risk and notify immediate supervisor of defective or missing items. Items determined to pose a safety risk are not to be used until they can be rendered safe and are to be tagged, isolated, or removed from service.
Who does the supervisor notify after becoming aware of a safety concern with the above example?

Accident and exposure (incident) investigations are performed to gather information on the cause(s) that contributed to their occurrence and to prevent reoccurrence.
Events that are to be investigated include:
-all work related injuries and illnesses
-vehicle accidents
-near miss incidents with potential for significant injury
“Serious injury or illness” is defined as any work related injury or illness which requires inpatient hospitalization for a period in excess of 24 hours for other than medical observation, or in which an employee suffers a loss of any member of the body or any serious degree of permanent disfigurement.
These types of incidents must be reported to Cal/OSHA as soon as practical but not longer than _____ hours after knowledge of the fatality, or serious injury or illness.

Accident/Exposure Investigation/Near Miss Investigation
Station/Company Commanders Responsibilities:
-Review Form 5020 and an Accident Investigation Form (and any other accident reports) and maintain records.
-Ensure that corrective actions are taken to prevent reoccurrence.
-Verify that the Civilian Department Manager has been notified of an injury or illness.
-Provide the injured employee with Workers’ Compensation Claim Form (DWC 1) and Notice of Potential Eligibility within 24-hours of receiving notice of knowledge of injury. If the employee is off work, the form may be mailed or delivered in person.
Record Keeping Retention:
The written IIPP - Indefinitely
Completed Inspection and Abatement Forms - 1 year
Completed Investigation - 5 years
Employee Training Records – 1 Year
Cal/OSHA 300 and 300A Forms - 5 years
Medical and Exposure Records - Duration of employment plus 30 years
