Why is nutrition important for healing?
glucose/ protein vital for regeneration/ growth
not enough energy sources- breakdown of vital
increased demand- inactive/dormant wound
What is the initial assessment?
determines the severity, treatment and facilities needed for wound healing
- rapid approach- bacteria
What is the Golden Period?
treating open wounds within first 6 hours
What are Lavage (cleaning) considerations?
GA
fill wound- water soluble gel- prevents embedded hair
flush- copious amounts, disloging bacteria
rehydration of necrotic tissue- aids debridement
What is the aim of lavage?
removes debris/ dilute bacteria load- bacterial biofilms- coating over wound
What are the 3 different types of lavage fluids?
sterile, non-sterile and isotonic fluid (cell osmotic pressure)- reduces fibroblast damage
What is Debridement?
removes all devitalised, contaminated/ infected tissue/ debris
reduces macrophage work
speeds healing
closer examination of wound
Non-Surgical Debridement-
wet to dry dressings- mechanical
hydrogels/ honey- rehydrates/ loosens dead cells- drawing toxins from base of wound into dressing
dirty- several weeks
sterile medical grade maggots- unhealthy tissue (costly)
Surgical Debridment-
many wounds need this to remove gross contamination
repeated- tissues devitalise
exploration
change sterile equipment- wound cleaner
When should antibiotics not be seen?
as an alternative lavage/ debridement
When should culture and sensitivity be preformed?
exudative wounds- systemic signs
What are examples of synergistically bacteria?
aerobes/ anaerobes
What is important when checking wound healing progression?
wound exudes less
no smell
wet?
granulation- bright red
new epithelial tissue growing from wound edges
What are the layer stages of dressing?
primary
secondary
teritary
Primary-
absorbent/ non-absorbent
adherent/ non-adherent
passive/ interaction/ bioactive
Secondary Considerations-
padding, 50% overlap
no overstretch- causes pressure sores
Tertiary Considerations-
protects other layers of bandage- soiling/ mutilation
easily elastic/ cohesive
