What are signs of testicular cancer?
- Lump on either testicle (refer)
- Enlarged testicle
- Dull ache in abdomen/groin
- Gathering of fluid in scrotum
- Heaviness in scrotum
What is the cause of alopecia + how do you manage it?
- Male pattern baldness
- Effect of DHT on over sensitive hair follicles
- Minoxidil (P), Finasteride 1mg tab (POM)
What are causes of snoring?
- Obstruction
- Loss of muscle tone
- Fat deposits/weak chin
- Collapsing nostrils
- Deviated septum
- Large soft palate/uvula
How do you manage snoring?
- Less alcohol + stop smoking
- Nasal spray + dilators
- Oral shield + chin up straps
What are symptoms of benign prostatic hyperplasia?
- Incomplete emptying
- Inc frequency
- Intermittent flow
- Urgency
- Weak stream
- Nocturia
What do you give to treat benign prostate hyperplasia?
Flomax MR relief 400mcg (tamsulosin)
- 45-75
- Symptoms present 3 months + more
- 2 wk initial supply
- Improvement = further 4 wks
- See GP w/in 6 wks
What are the counselling points for benign prostate hyperplasia?
Red alcohol, caffeine, weight + fluid intake before bed
When would you not supply tamsulosin?
- Symptoms <3 months
- Prostate surgery
- Undiagnosed diabetes (thrush, UTI)
- Liver, kidney + heart problems
- Postural hypo
- Cataract surgery
- Cloudy vision
What is the treatment for erectile dysfunction?
Sildenafil 50mg
- 50mg 1hr before intercourse
- Sexual stim needed
- Empty stomach
- S/e: headache, flushing, visual disturbance. nausea
What are the CI/red flags for sildenafil?
- Female
- <18
- Don’t have ED
- Premature ejaculation
- Hypotension
- Hepatic impairment
- Severe renal impairment
- Nitrates, alpha blocker
- MI/stroke in previous 6 months
- Severe HF
What are the symptoms of thrush?
- Irritation, burning around penis + under foreskin
- White discharge
- Unpleasant smell
- Difficulty pulling back foreskin
What are the requirements to treating thrush?
Symptomatic AND sexual partner diagnosed
What is the treatment for thrush?
- Fluconazole 150mg caps
- Clotrimazole 2% cream: BD/TDS upto 2 wks
What are the red flags for thrush?
- Partner has no candida infection
- <16 + >60
- Diabetic
- 2 episodes in 6 months
- Systemic infection
- Hx STI
