Week 118 Pyrexial Illness Flashcards
What factors do you take into account when choosing an antibiotic?
Likely organisms Current condition Previous treatment Allergies Patient condition Age Pregnant/breastfeeding
Name four intracellular bacteria
Legionella
Chlamydia
Mycobacterium
Listeria
Which antibiotics pass into mammalian cells?
Glycopeptides
Macrolides
Quinolones
Tetracyclines
Which antibiotic drug accumulates in liver impairment?
Rifampicin
Which antibiotic should you NOT drink with?
Metronidazole
Which drugs do you not use in renal impairment?
Aminoglycosides
Glycopeptides
Which class of antibiotics do you NOT use with statins?
Macrolides
What type of medication is a macrolide?
Bacteriostatic
Similar spectrum to Amoxicillin
Active against G -ve and G +ve
Use for PENICILLIN ALLERGIC PATIENTS
Which antibiotic would you use for a UTI?
Good oral absorption
Can be used for UTI prophylaxis
Side effects are rare
Trimethoprim
What is methotrexate toxicity?
Both trimethoprim and methotrexate reduce folate metabolism, leading to additive effects.
You end up with liver toxicity, bone marrow suppression, neutropenia, and infection.
What are the pharmacokinetics of macrolides?
Erythromycin is destroyed by stomach acid (enteric coating)
Iv administration causes phlebitis
Absorption of azithromycin and erythromycin reduced by food.
Inhibits hepatic metabolism of theophyline, warfarin and statins.
Clarythromycin excreted by kidneys (reduce dose in renal impairment)
What do you do with Clarithromycin in renal impairment?
Reduce the dose.
Which antibiotics do you not use when you’re on Statins?
Macrolides
