What is volvulus?
A
rotation of a loop of bowel around the axis of its mesentery that results in bowel obstruction and potential ischaemia

Where do most volvulus occur?
A
- Sigmoid colon 65%
- Caecum 30%

What are RF for volvulus?
A
- Long sigmoid colon
- Long mesentery
- Mobile caecum
- Chronic constipation
- Adhesions
- Parasitic infections
- Neonates: malrotation

What are symptoms of volvulus?
A
- Severe colicky pain
- Vomiting
- Abdo distention and tenderness
- Signs of dehydration
- Absent/tinkling bowel sounds
- Constipation

What is the management of a sigmoid volvus?
A
- Sigmoidoscopy with detorsion
o If no suspicion of ischaemia or perforation
o 75-95% success
o Recurrence 84% - need to do surgery after detorsion to prevent

What is the management of a caecal volvulus?
A
- Colonoscopy and detorsion not done – risk of perforation
* Need to perform surgery
