cancers arising in the bile ducts either intrahepatic or extrahepatic
What are RF for cholangiocarcinoma?
- advancing age
- male
- smoking
- obesity
- chronic inflammation of bile ducts
What may you see on the abdo exam for cholangiocarcinoma?
1 Virchow’s Node
2. Sister Mary Joseph nodule
What are the other symptoms of cholangiocarcinoma?
- FLAWS
2. RUQ pain
What are signs and symptoms of biliary obstruction (extrahepatic only)?
- Jaundice (painless)
- Palpable gallbladder
- Pruritus
- Pale stool, dark urine
What could chronic inflammation of the bile duct be due to?
- PSC
- cirrhosis
- bile duct stones
- infection – parasitic liver disease (Clonorchis sinensis liver fluke in SE Asia)
What is sister mary joseph nodule?
periumbilical lymphadenopathy
What is Virchow’s node?
supraclavicular lymphadenopathy
What is the gold standard investigation for cholangiocarcinoma?
ERCP with biopsy
What bloods are done for cholagniocarcinoma?
- LFTs
- clotting studies
- CA19-9
- CEA
What other imagining is done for cholangiocarcinoma?
- Imaging – USS, MRCP
2. Radiography – PTC
What is the management for cholangiocarcinoma?
- Patients often present late which limits the scope for surgery
- Surgery
- Adjuvant chemotherapy or radiotherapy can prevent recurrence
What type of surgery is available for cholangiocarcinoma?
- Removal of bile duct – small and localised tumour
- Partial hepatectomy – intrahepatic
- Whipple’s procedure – for distal bile duct tumours
What would blood results show for cholangiocarinoma?
- raised ALP, GGT and BR

What does the USS allow?
identification of obstruction and dilation
