What is IBS?
chronic condition characterised by abdominal pain associated with bowel dysfunction
What is typical epid for IBS
10-15% of population, mostly female and less than 50yo
What are the risk factors for IBS?
- Women
- <50 years old
- Previous enteric infection
- History of physical or sexual abuse
What are symptoms or signs of IBS?
- Abdominal discomfort (generalised cramping)
- Alteration of bowel habits associated with pain: relief when passed
- Abdominal bloating or distension: improves with passing stool or flatus
- Normal examination of abdomen
What are potential differential diagnosis of IBS?
- Crohn’s disease
- Ulcerative colitis
- Lymphocytic and collagenous colitis (microscopic colitis)
- Coeliac disease
- Colon cancer
- Bowel infections
- Non-coeliac gluten sensitivity
- Bile acid malabsorption
- Small bowel bacterial overgrowth
What is the 1st line investigation for IBS?
FBC
What does FBC show?
normal; anaemia suggests non-IBS disease
What other investigations may you do?
-exclude IBD, coleiac and colonic cancer
What is key symptoms of iBS?
Defecation RELIEVES abdominal pain/discomfort (good q for gastro history)
What are 3 different types of IBS?
IBS-D: IBS with diarrhoea
IBS-C: IBS with constipation
IBS-M: IBS mixed type
What is the lifestyle management of IBS?
- Fibre
- Avoid: caffeine, lactose, fructose.
- Stress management
- Education + Reassurance
- Probiotics ?
What is the medical management of IBS?
- Laxatives (IBS-C)
- Antispasmotics (cramps)
- Antidiarrhoeals (IBS-D)
