primary malignancy of hepatocytes
How common are hepatocellular carcinomas?
forms 90% of primary liver tumours (cholangiocarcinoma the other 10%)
What are RF for hepatocellular carcinoma?
- Cirrhosis due to hepatitis
- alcohol or drugs
- AI hepatitis
- Haemochromatosis
- NAFLD
- PBC
What are the signs and symptoms of hepatocellular cancer?
- Present late
- FLAWS
- Signs of chronic liver failure
- May present with decompensation
What are signs of chronic liver failure?
- Hepatosplenomegaly
- Pruritus
- Jaundice
What are signs of decompensation?
- Acute deterioration in liver function

What are the Ix for hepatocellular carcinoma?
- Screening for high risk groups
- Bloods – aFP tumour marker
- Imaging - USS
What are high risk groups?
patients with cirrhosis secondary to hepatitis or alcohol or haemochromatosis
What are you looking for in bloods for hepatocellular carcinoma?
aFP tumour marker
What is the management for hepatocellular carcinoma early disease?
- Surgical resection – non-cirrhotic
* Liver transplantation – cirrhotic
What is the management for hepatocellular carcinoma unsuitable for surgery?
- Percutaneous ethanol injection for peripheral lesions
* Sorafenib – multikinase inhibitor
What are possible complications of hepatocellular carcinoma?
- Liver failure
- Secondary mets
- Death
What are possible ddx of hepatocellular carcinoma?
- Cholangiocarcinoma
