What is ascites?
pathological collection of fluid in the peritoneal cavity
What are the most common causes of ascites?
- cirrhosis
- congestive heart failure
- Malignant ascites
What are the 2 types of non-peritoneal causes of ascites?
- Conditions that cause portal hypertension

What are examples of conditions that cause portal hypertension leading to ascites?
- cirrhosis
- alcoholic liver disease
- fulminant hepatitis (e.g., due to paracetamol toxicity)
- subacute hepatitis
- hepatic veno-occlusive disease
- massive liver metastasis
- congestive heart failure
- constrictive pericarditis
- Budd-Chiari syndrome
What are examples of conditions that cause hypo-alubminaemia that lead to ascites?
- Nephrotic syndrome

What are other non-peritoneal causes of ascites?
- myxoedema
- ovarian tumours (cancer, benign tumours and ovarian hyperstimulation syndrome)
- pancreatic ascites
- chylous ascites and biliary ascites
- urogenital surgical trauma
What are the malignant peritoneal causes of ascites?
- peritoneal mesothelioma
- peritoneal carcinomatosis
- ovarian cancer
What are the infectious peritonitis causes of ascites?
- tuberculosis
- Chlamydia trachomatis
- fungal and parasitic infections
What is an autoimmune cause of ascites?
SLE
What is primary peritonitis caused by?
spread of an infection from the blood and lymph nodes to the peritoneum and is rare
What is secondary peritonitis caused by?
infection from the gastrointestinal or biliary tracts
What are the common differential diagnosis in ascites?
- Hep C
- Alcoholic liver disease
- Congestive heart failure
- Nephrotic syndrome
- Pancreatitis
What investigations do you carry out in ascites?
- US
- Bloods
- Serological tests for hepatitis A, B, and C
- Aspirate ascitic fluid: cytology, culture and albumin
What is grade I-III of US ascites?
- Grade 1: mild ascites detectable only by ultrasound
- Grade 2: moderate ascites with moderate symmetrical distention of abdomen
- Grade 3: large or gross ascites with marked abdominal distention
What scores can you use in ascites?
MELD + Child Pugh
