A
- AVM - GSL - GSL VMP
- Non-Food Producing Animals - VPS - Vet, pharmacist, QP, can supply without Rx
- POM - VPS - Oral/written Rx - written if supplier not prescriber
- POM - V - prescribed by vet on written Rx, pharmacist adn Vet can supply

- Written Rx, signed by prescriber
- Supply made to persons name on Rx

A
- Advise safe administration
- Advise on contradiction/ warning
- Prescribe/ supply MIN. quantity required

What are the Rx Requirements for POM-VPS and POM-V?
A
- Written Rx valid for 6 months - CD 1-4 = 28 days
- Prescriber’s name, address, tel, qualifications
- Date
- Owner’s name and address
- Identification and species of animal
- Address of animal - if different
- Prescriber’s signature
- Name of drug, quantity, dose, admin instructions - as directed - unacceptable
- Warning/withdrawal period if necessary
- ‘For administration under the cascade’

What is the validity of CD Rx?
A
Valid for 28 days CD 1-4

What are the Vet CD Rx requirements?
A
- Name and address of prescriber
- RCVS reg no. of prescriber
- Name and address of recipient
- Name of drug, form, strength, quantity in words and figures
- Statement that the item is prescribed for an animal/herd under the vet’s care
- For instalment dispensing - quantity and interval

Repeats of Sch 2-3 CDs is not allowed

How long can you keep Rx record for and what are their requirements?
A
- Keep Rx record of transaction for 5 years
Requirements: - Date of receipt/ supply
- Name of VMP
- Batch No.
- Quantity
- Name and address os supplier/ recipient
- If supplied on Rx - name and address of prescriber

What are the labelling under the Cascade requirements?
A
- Name and address of pharmacy
- Name of prescriber
- Name and address of owner
- Identification and species of animal
- Date of dispensing
- Name, dose, admin instruction
- Exp date
- Special storage
- Necessary warnings
