  1. AVM - GSL - GSL VMP
  2. Non-Food Producing Animals - VPS - Vet, pharmacist, QP, can supply without Rx
  3. POM - VPS - Oral/written Rx - written if supplier not prescriber
  4. POM - V - prescribed by vet on written Rx, pharmacist adn Vet can supply

What are the Rx Requirements for POM-VPS and POM-V?

A
  • Written Rx valid for 6 months - CD 1-4 = 28 days
  • Prescriber’s name, address, tel, qualifications
  • Date
  • Owner’s name and address
  • Identification and species of animal
  • Address of animal - if different
  • Prescriber’s signature
  • Name of drug, quantity, dose, admin instructions - as directed - unacceptable
  • Warning/withdrawal period if necessary
  • ‘For administration under the cascade’
