What is DMS?
Referral for selected patients to pharmacy for extra guidance post discharge giving advice for new meds or change made by hospital.

What is Stage 1 DMS?
Within 72 hours of referral
- Pharmacist check changes, monitoring requirements and contraindication.
- Record made to alert staff to conduct stages 2 and 3 when received Rx
- Check any previously ordered Rx
What is Stage 2 of DMS?
Once Rx received
- Ensure changes made
- GP/PCN alerted of any issues
- Record made
What is Stage 3 of DMS?
On collection of 1st Rx
- Discuss changes
- Establish they understand what each medicine is for
- Educate how to take - compliance
- Communicate important info to go
- Offer to dispose of any unwanted medicines
- Offer NMS If appropriate
