UTI in pregnancy inc risk of
preterm delivery
asymptomatic bacteriuria
bacteria present in urine w/o infective symptoms
testing for asymptomatic bacteruriua
at booking and routinely throughout pregnancy
microscopy, culture and sensitivites
lower UTI presentation
dysuria suprapubic pain inc frequency urgency haematuria
pyelonephritis presentation
fever loin, suprapubic, back pain vomit malaise loss appetite haematuria
urinalysis - nitrites
gram negatie bacteria - e.coli
Mx
UTI in pregnancy requires 7 days of antib
nitrofurantoin
amoxicillin (only after sensitivities known)
cefalexin
nutrofuratoin use in pregnancy
needs to be avoided in 3rd trimester - risk of neonatal haemolysis
trimethoprim use in pregnancy
avoid in 1st trimester as works as folate antagonist - risk congenital malformations esp neural tube defects
generalyy avoided unless necessary
