Acute Fatty Liver of Pregnancy Flashcards
1
Q
when does it occur
A
3rd trimester of pregnancy
2
Q
what happens
A
rapid accumulation of fat within hepatocytes causing acute hepatitis
3
Q
why does it happen
A
impaired processing of fatty acids in the placenta - result of genetic condition of fetus that impairs fatty acid metabolis
4
Q
most common cause
A
long-chain 3-hydroxyacyl-CoA dehydrogenase (LCHAD) deficiency
autosommal recessive
5
Q
LCHAD emzyme
A
important in fatty acifd oxygation - breaking down fatty acids to be used as fuel
6
Q
LCHAD enzyme deficiency
A
placenta and fetus unable to break down fatty acids –> they enter maternal circulation and accumulate in liver
7
Q
presentation
A
malaise fatigue N&V jaundice abdominal pain lacka appetite ascites
8
Q
blood test results
A
elevated liver enzymes raised bilirubin raised WCC derainef clotting low platelets
9
Q
Mx
A
prompt admission and delivery of baby
most pt recover after delivery
