What bacterial factors can increase risk of UTI?
Fimbriae - allow attachment to epithelium
Haemolysins - damage host membranes
K antigen - production of polysaccharide capsule
Urease - breaks down urea = favourable growth environment
What is the most common UTI causing bacteria?
E.coli
What is cystitis?
Lower UTI
What are the symptoms of cystitis?
Dysuria
Frequency
Urgency
What is pyelonephritis?
Upper UTI
What are the symptoms of pyelonephritis?
Fever
Loin pain
Dysuria
What is an uncomplicated UTI?
Infection by a usual organism in a patient with a normal urinary tract and normal urinary function
What is a complicated UTI?
UTI when one or more factors are present that predispose the person to persistent/recurrent infection
What can cause a complicated UTI?
Abnormal urinary tract
Virulent organism
Impaired host defences
Impaired renal function
How are UTIs investigated?
MSU
Urine dip
Urine culture
How is cystitis managed?
Trimethoprim
3 days = uncomplicated
5-7 days = complicated
How is pyelonephritis managed?
14 days co-amoxiclav
