Common Accidents in long term care
falls (most often during shift change) failing to identify resident before care is given burns poisoning choking cuts & abrasions
Fall risk because
new environment difficulty walking weakness poor vision confusion medications
Fall prevention
Identify at risk persons & report to nurse Respond to call lights promptly Wipe up spills Remove clutter from walkways Side rails up if ordered Proper fitted clothing & non skid shoes Never leave resident alone in shower or bath Report loose handrails, tiles, carpet immediately Call light in reach Offer bathroom frequently Report lighting problems Eyeglasses on
If resident falls
use body to slide to floor
Don’t try to stop or catch them

Call light in reach Side rails up Bed at lowest Cranks in neutral Lock bed
Restraints
vest, wrist, or ankle
side rails
geriatric chairs (only with tray)
some medications
Guidelines for Restraints
Must have drs order
Cannot be used for discipline or convenience
Only used to prevent harm to self & others AFTER other methods fail
Restrained person needs to be monitored
check every 15 minutes
remove every 2 hours for 15mns for food, toilet, check skin
apply with quick release knot
