Is smoking good or bad for UC?
Smoking protects against ulcerative colitis
Predominant symptoms in the history for UC?
Rectal bleeding +/- PR mucus.
Where does the inflammation start in UC?
Starts in the rectum (proctitis)
Inflammation in UC?
Superficial ulcers
Goblet ells.
Acute cryptitis and crypt abscess
What layers does the inflammation involve?
Superficial, mucosa and submucosa onlu
Extra intestinal manifestations?
PSC Cholangiocarcinoma Erythema nodosum Finger climbers Ulcers Stomatitis. Seronegative arthritis
How do you classify severity?
True love and witts criteria?
What is mild severity?
< 4 bowel movements, small amount of PR bleed, apyrexial.
Moderate severity?
4-6 bowel movements.
Moderate PR bleed.
Slight temp
Pulse 70-90
Severe ?
> 6 bowel movements and signs of systemic upset.
E.g. anaemia, high temp, pulse > 90, ESR > 30.
Appearance of colon in UC?
Lead pipe.
Diagnostic test for UC?
Flexible sigmoidoscopy
Assessing extent?
Colonscopy.
Main complication of UC?
Toxic megacolon.
Total but non-obstructive dilatation of colon.
AXR showing colon > 6cm. Thumb printing and loss of Haustra.
What is given for a severe flare of UC?
IV methylprednisolone.
