Typhoid fever ETIOLOGIC AGENT
Salmonella typhi

Feces and urine of infected persons
PS: Ulceration of peyer’s patches that causes emboli to the skin (rose spots)

1-3 wks
Typhoid fever Mode of transmission
● Direct or indirect contact; feces/urine
● 5Fs
● Contaminated water
Typhoid fever S/sx
S, H, R, U, L, S
● Step like or ladder like fever
● Headache
● Rose colored spots on the trunk , chest and abdomen
● Ulceration of peyer’s patches
● Loss if appetite
● Splenomegaly
Typhoid fever Dx tests
W, C (WelCome)
● Widal Test/Typhidot; test for agglutination (the antigen-antibody response will clamp together)
● CBC; expect leukopenia
Typhoid fever Nursing Management
● TSB to promote heat loss
● Oral care
● Report blood in the stools

● Chloramphenicol
● Cotrimoxazole
● Ampicillin
Typhoid fever PATHOGENESIS
- Ingestion of contaminated food/water
- Attach to Peyer’s patches in terminal ileum
- Incubation stage
- Pass through lymphatics to reticuloendothelial system (Pass into circulation= abscesses and osteomyelitis)
- From gallbladder secreted back into GI tract (Cholecystitis)
- Necrosis and ulceration of Peyer’s patches
- Intestinal perforation and bleeding
