Q

Mode of transmission?

A

Classic: immature oocysts - from cat faeces (classic)
mature oocysts - contaminated food

But more commonly pig meat containing bradyzoites (undercooked)

Q

If immunocompetent person gets toxo, symptoms?

A

10% to 20% of patients with acute infection may develop cervical lymphadenopathy and/or a flu-like illness

Mostly asymptomatic!

Rare: ocular damage, visual disturbances

Q

Diagnosis of toxo?

A

14
Q

What should you do if you get a positive IgM antibody for toxo (IgG negative)

A

Repeat in 2 weeks - could be fast positive or acute infection
IgG should be positive 2 weeks later if acute infection.
IgM can sometimes be present for up to 2 yrs

16
Q

Positive IgG toxo but negative IgM, meaning?

A

Infected with Toxoplasma for six months or more.

17
Q

2 stages of life cycle that are infective?

A

Cysts with bradyzoites from raw meat
Oocysts with sporozoites from cat faeces

18
Q

Of those that get infected, how many have symptoms of toxoplasmosis?

A

10%

19
Q

Symptoms of reactivation of toxo vs acute infection

A

Chorioretinitis vs
Acute: mononucleosis like syndrome with lymphadenopathy

Rarely presents with myocarditis, pericarditis, hepatitis

20
Q

How does toxo present in pregnancy? Risk to fetus?

A

Recurrent abortions
Congenital CNS and eye disease

21
Q

How does toxo in eye present?

A

CR: Painless visual loss
Can also see a scar

22
Q

Toxo in CNS in immunocompromised patients?

A

Diffuse meningoencephalitis and isolated cerebral lesions - single, ring enhancement

23
Q

Rx of CNS Toxo?

A

Dexamethasone plus Pyrimethamine plus sulfadiazine PLUS FOLINIC ACID (NOT FOLIC)

24
Q

Symptoms of toxo in HIV?

A

Focal signs, headache, seizures, confusion

25
Q

SEs of pyrimethamine and sulfidiazine, how to prevent?

A

Anaemia, leukopenia, thrombocytopenia
FOLINIC ACID

26
Q

First 20 weeks of pregnancy with acute toxo Rx?

A

Spiramycin then switch

27
Q

Rx of a neonate with congenital toxo?

A

Pyrimethamine + Sulfadiazine + folinic acid

28
Q

Primary prophylaxis vs secondary prophylaxis of toxo in immunocompromised host?

A

Primary: co-trimoxazole
Secondary (had toxo Rx): continue pyrimethamine and sulfadiazine
