What is toxic shock syndrome?
Septiceamia from toxin (TSST1) produced by staphylococcus and streptococcus
Straph- exotoxin (TSS toxin 1)
Streptococcus- inflam cascade initiation
aetiology- multisystem inflam response to bacterial exotoxin
RF-
tapons that absord more- infrequent change, overnight use of tampons
still rare -17 per 100000 tampons per year
Sx and Ix of toxic shock syndrome
Sx-
fever (high), myalgia, diffuse red patch, D&V, sore throat, headache,
SHOCK
ix- FBC- raiseed WCC, low platelet
UE- impaired renal, LFT, raised CK, CRP
micro- High vaginal swab, Blood cultuure, tampon cultur
Mx and complications of toxic shock syndrome
ABC approach and remove tampons
abx- broad spectrum
complications
septic shocl, DIC, ARDS, death
prog- death is 5-15%
