Candidiasis Flashcards
What is candidiasis (aetiology and RF)
Thrush - caused by candida albicans (95%) or candida glabrata (5%) –
can be spontaneous or 2nd ary to disruption of vaginal flora (2nd most common after BV)
can be oral - or invasive (if in sterile sites)
RF -
oestrogen exposure (pregancy, reproductive years), diabetes/SGLT2 inhbits
sex
Sx and Ix of candidiasis
Sx- clinical are vulval itching, soreness, irriration, cottage cheese discharge
No ix neecessary really but
pH <4.5, microscophy - gram +ve spores/hyphae
should check MSU (UTI) and HbA1c)
Mx and complications of candidasis
1st line - clotrimazole pessary (small ring to insert in vgina) +1% clotrimazole cream (BD)
2nd line/sevre - oral fluconazole
advice- avoid tight fitting synthetic clothes, avoid perfume/irritants, do not wahs with soap, dont douche
called reccurent if 4> proven sx episodes
tx with induction and maintenance of fluconazole
pregnant -only topical
complications - heptatotoxic from systemic azole therapy
Oestophageal/oral candidasis in immunocompromise
