What is Lichen sclerosus? (RF, Aetiology)
Chronic inflammatory skin condition- usually affecting genital (vulval) and perineum
not contagious, and occurs at age extremes (under 10, over 60)
aetiology - autoimmune- Hypothyroid, graves, T1dm
genetic
Sx and Ix of Lichen sclerosus
Sx – “white polygonal papules”
Hypopigmentation/atrophy
Pruritis (itch)
White/shiny vuvla in “figure of 8 pattern”
Dyspareunia and Dysuria (tightened skin)
CAN be on wrist, upper trunks, breast, neck, armpits
Ix - clinical only
Mx and complications of Lichen sclerosis
Good skin care/conservative/moisiturise
1st line (3 monhs 0- clobesatole propionate (steroids ointment)
if resist-
2nd line -topical tacrolimus + biopsy
complications - never fully cured, squamous cell cancer
