Thrombotic Disorders Flashcards
Most common hereditary thrombotic disorder:
Factor V Leiden
VERY rare in non-caucasians
Dx fro factor V Leiden?
genetic testing
**point mutation in factor V gene
What’s the problem with defective factor V?
Can’t be cleaved by protein C
Natural anticoagulant
inhibits IIa, VIIa, IXa, Xa, XIa
potentiated by heparin
antithrombin III
Warfarin induced skin necrosis; think….
Protein C or S deficiency
**gets knocked out before clotting factors
inactivates Va and VIIIa
promotes t-PA action
keeps cytokines low
protein C
thrombotic state + vascular injury = ?
Purpura fulminans
Super rare, same as protein C def?
protein S def
Mutated gene makes too much thrombin:
Factor II mutation
rare in non-caucasians
What is factor II?
prothrombin
MTHFR gene mutation?
Homocysteinemia
To bad things about homocyteine with regardds to thrombosis:
forms ROS –> endothelial damage
interferes with NO (vasodilator and antithrombotic)
Other way to get homocysteinemia besides MTHFR?
B12/folate deficiency
**less worrisome
Falsely prolonged INR
autoantibody involvement
Antiphospholipid Antibodies
Inhibitors (antiphospholipid antibodies) in vivo _________ coagulation.
