WHEN DO YOU DISCHARGE?
When the client has met their goals
When the client has reached their MAX POTENTIAL
When the client is ready to transition to another level of care
When there is a significant MEDICAL DECLINE affecting the client’s ability to participate
When the client misses 3 consecutive visits (varies by setting and circumstance)
When the client expires
