Traumatic Brain Injury
Mainly in young men. Not all the same, difference is open and closed skull injury
Open skull fracture
Primary damage: Bones are cracked like gunshot
Closed skull fracture
Secondary damage: Focal atrophy (coup a contrecoup). Brain moves forwards and gets damaged on impact on front as well as back (backfire)
Subdural hemorrhage
Where bleeding is between arachnoid mater and dura mater
Epidural hemorrhage
Bleeding between dura mater and skull
Subarachnoid hemorrhage
Bleeding is intracerebral. In white/grey matter (delayed effects)
Secondary damage
Oedeem: Zwelling door beschadiging bleodvaten
Hypoxie: too little oxygen
EVM schale
Schaal met 3 onderdelen voor testen schade:
- E score, opening of eyes (4)
- M score, motor reaction (6)
- V score, verbal response (5)
8 or lower is severe. (till 12 intermediate)
Commoto and contusio cerebri
Onderscheid tussen lichte en zware hersenletsels. Grens is 15 min buiten bewustzijn en/of 60 min PTA van overgang licht naar middel. 1-7 dagen PTA is zwaar
