A

LIIPSFREUD

• Left unfixed or unrefrigerated for an extended period

• Incomplete patient information/clinical history

• Insufficient sample for processing

• Putrefied or autolyzed specimen

• Spilled or contaminated specimens

• Failure to the requesting physician to enter the request in the computer

• Referral pathology consultation material without Histopathology report of referring hospital.

• Enter containers without the specimen or form

• Unlabeled or wrong label on the specimen

• Damage specimen or broken slide.

  1. Complete forms/documents such as Histopath Request Form, Receipt of transaction and other pertinent documents.
  2. Specimen pre-fixed with formalin (done before the gross examination)
  3. Accession of the specimen by writing the serial number
  4. Write all the details in the logbook (serial number, Px’s name, age, gender, room, specimen, name of pathologist who will examine, date specimen received, due date).
  5. Fill up the surgical pathology worksheet
  6. Attached all documents in one folder.
