What is septic arthritis?
Infection of 1 or more joints caused by pathogenic inoculation of microbes
How quickly can septic arthritis destroy a joint?
Under 24h
Which joint is affected in >50% of cases?
The knee
What is the first investigations?
Urgent joint aspiration for synovial fluid microscopy and culture, blood cultures
What other investigations would you do?
CRP/ESR (raised)
What is the main differential diagnosis?
Crystal arthropathies
What management should you start if in doubt?
Empirical IV antibiotics until sensitivities are known - flucloxacillin
When should you consider septic arthritis?
Any acutely inflamed joint
What are the predominant organisms responsible?
Staphylococci, streptococci
What are risk factors for septic arthritis?
Pre-existing joint disease, joint prostheses, IVDU, immunosuppression, alcohol misuse, diabetes, intra-articular corticosteroid injection, recent joint surgery
How does septic arthritis present?
Hot, swollen, painful, restricted joint, fever
