What is osteoarthritis?
Non-inflammatory degenerative progressive destruction of cartilage from repeated mechanical forces
What are some risk factors for OA?
High intensity labour, age
What joints are usually affected by OA?
Knee and hip
How does OA often present?
Painful joints stiff for 30 or less in the morning and gets worse throughout the day. Distal Heberden nodes. Proximal Bouchard nodes
What joints are usually affected by OA in post-menopausal females?
DIP, PIP, CMC and knees
What X-ray findings are typical of OA?
LOSS Loss of joint space Osteophytes Sclerosis Subchondral cysts
What does sclerosis look like?
Whiter areas
What general treatment can you do?
Exercise to improve local muscle strength and general aerobic fitness, weight loss if overweight
What analgesia will you give to start?
Regular paracetamol +/- topical NSAIDs
What injection can you give?
Intra-articular steroids
What non-pharmalogical treatment would you start?
Physiotherapy, occupational therapy
What surgical option is there?
Joint replacement
What are potential complications?
Destruction of the joint and loss of function
