- kristal —> sterke inflammatie
- kan vanzelf oplossen
- Je kan de kalk ook weghalen
- Supra-acromiaal pijnstilling inspuiten

Rotator cuff impingement
A
- Insertie van de rotator cuff in conflict brengen met de distale hoek van het acromion
- Ook nog een AC gewricht en ligamentaire structuur die voor verdere inkneling kunnen zorgen
- Er is een vicieuze cirkel: pijn —> minder gebruiken —> zwakkere rotatorcuff —> schouder naar proximaal —> meer impingement
- Painful arc vanaf 60° à 70° tot 120° pijnlijk bij het abduceren van de arm
- Neer test -> arm naar voor bewegen is pijnlijk
- Hawkin’s test —> arm naar voor en je duwt die in passieve endorotatie => heel veel pijn

Behandeling van rotator cuff impingement

- Infiltratie
- Tonificatie van de endo en exorotatoren
- Chirurgie - arthroscopie, bursectomie, acromioplastiek, AC resectie

Supraspinatus of infraspinatus scheur
A
- Sommige mensen hebben een scheur zonder last te hebben door compensatie
- Klassiek starten de scheuren: supraspinatus insertie —> infraspinatus insertie —> anterieur in de subscapularis
- Bewegingsdeficit: geen actieve abductie, passieve wel nog
- Vaak is de lange bicepspees de eerste => een intraarticulaire pees —> Popeye sign
Testen:
- Jobe test & empty can supraspinatus
- Exorotatie van infraspinatus

Klinische tekens:
- Belly press, Napoleon

Chirurgisch herstel:
- Subscapularis hechting
- Spier-pees transfer => pectoralis maior
