- Bronchodilator
- Sympathomimetic
Explain the pharmacodynamics of Salbutamol
- Selective β2 stimulation resulting in bronchodilation and some degree of vasodilation
- Some β1 effect – especially with repeated doses
- Little or no α stimulation
What is the onset timing for nebulized Salbutamol?
5 minutes
What is the peak timing for nebulized Salbutamol?
1.5-2 hours
What is the duration for nebulized Salbutamol?
3-8 hours
What are the indications for Salbutamol?
- Bronchospasm associated with asthma, bronchitis or emphysema
- Bronchospasm & wheezing secondary to other causes, such as anaphylaxis
What are the contraindications for Salbutamol?
- Hypersensitivity
- Hemodynamically significant tachyarrhythmias
List the precautions for Salbutamol
- Coronary disease (increased MVO2)
- COPD patients with degenerative heart disease
- Diabetes (decreases the effectiveness of insulin)
List the adverse effects for Salbutamol
- Restlessness, apprehension, fear, weakness, vertigo
- Nausea and vomiting
- Tachycardia, dysrhythmias
- Paradoxical worsening of respiratory distress, - pulmonary edema
- Sweating, pallor, flushing
What is the ADULT dosing for Salbutamol?
5 mg in 5 mL NS or H2O nebulized (with O2 at 6-8 L/min.) - repeat doses back to back prn - watch for signs of toxicity
What is the PEDIATRIC dosing for Salbutamol?
- Age < 1 year 2.5 mg via nebulizer
- Age > 1 year 5.0 mg via nebulizer
Describe signs of Salbutamol toxicity
H.R > 150 (>200 in pediatric patients) or if severe tremor or ventricular dysrhythmias develop
What is the MDI dosing for adult patients?
4 x 100 mcg dose via MDI. Repeat as required. No max dose
