what is the basis for respiratory exam?
general observation, hands, face, palpation, auscultation, end pieces
what is the general observation?
of the patient: colour, respiratory difficulties, audible breath sounds, position of the patient and their general habitus
bedside: oxygen, inhalers, nebulisers and check if have any pain
what do you check in hands?
clubbing, peripheral cyanosis, tar staining, cap refill, co2 flap for 30 seconds, respiratory rate and pulse
what do you look for in face?
general colour, conjunctiva pallor, peripheral and central cyanosis, JVP, lymphadenopathy
what nodes are palpated in lymphadenopathy?
supraclavicular, submandibular, submental, anterior and posterior cervical pre and post auricular and occipital
what do you look for in chest inspection?
chest wall deformities and scars all over
what is done in palpation of the chest?
locate the trachea, and apex beat. measure chest expansion, tactile vocal resonance, percussion of all lung areas
where do we auscultate?
J shape front and L shape back, and vocal resonance
what do we finally do?
check for sacral and peripheral oedema
what are the final pieces?
PEFR, BP, SPO2, sputum, temperature
how do we assess vocal resonance?
say 99 everytime the stethoscope is on - front and back, and tactile - use lateral border of hand on front
