What is Clomiphene?

A

SERM

Used to treat infertility due to anovulation

Antagonist at estrogen receptors in hypothalamus
Prevent normal feedback inhibition–> increase release of LH & FSH from pit–> OVULATION

May cause hot flashes, ovarian enlargement, multiple simultaneous pregnancies, visual disturbances

A

Estrogen and Progestins inhibit Lh/FSH–>prevent estrogen surge

Progestins–> thickens cervical mucus which cock blocks sperm from uterus
–>Inhibit endometrial proliferation so not as suitable for embryo

Contraindicated in smokers >35 y/o (incr risk of cardiovasc events); pts with history of thromboembolic events and stroke, or estrogen dependent tumor

Inhibits steroid synthesis and specifically inhibits:
17,20-desmolase and 17a-hydroxylase

Also used to prevent hirsutism in polycystic ovarian syndrome

Used for BPH by inhibiting smooth muscle contraction

Selective for a-1A,D which are only found in prostate vs vascular receptors (a-1B)

A

Erections baby

Block Phosphodiesterase 5–> Incr cGMP–> Smooth muscle relaxation in corpus cavernosum–> blood flow–> PENILE ERECTION

SE: HA, flushing, dyspepsia, IMPAIRED BLUE GREEN VISION
(think blue balls)

Risk of life threatening hypotension if use nitrates bc they do the same thing
