A

Train Reporting Number.
Where and when incident occurred.
Wagon types and numbers.
Position of wagons on the train or siding.
as much information about the incident as possible.
if any personnel or members of the public are involved.
the Emergency code (UN number and Alpha code: 4 numbers & 2 letters).

If you receive a report of an incident with an irradiated fuel flask (Nuclear) train, what 5 questions must you ask the train crew?

A

1 - Is the flask wagon derailed?
2 - Has the flask wagon been involved in a collision?
3 - Is there fire near the flask?
4 - Are large quantities of petroleum, LPG or other flammable liquid present?
5 - Is there visible damage to the flask or to its cover?

If yes to any of these, this information MUST be passed to Ops Control - in the same order as given above.
