Pulmonary Flashcards
1 yo with acute onset coughing, right sided expiratory wheeze. Next best step?
Airway fluoroscopy. (This age can’t cooperate with inspiratory and expiratory films!)
What is a positive sweat test?
60 mEq or greater
Carrier rate of CF in general population
1/25
Most common post-operative complication in patients with severe obstructive apnea
Pulmonary edema
Causes of transudate pleural fluid
Cirrhosis, CHF, Nephrotic syndrome
Exudate pleural fluid. LDH levels? Protein level?
LDH will be 2-3 times the serum level, and protein will be >3 g/dL
Treatment of active TB
2 months of RIP, then 4 months of INH and Rifampin. Could also do 9 months of rifampin and INH
Treatment of TB meningitis
Steroids + RIP x 2 months + Streptomycin, followed by 10 months rifampin and INH.
Child with pneumonia on XR. What is best diagnostic study to confirm?
Blood culture
What shifts oxygen dissociation curve to the right? (higher affinity for O2)
Increased temp, pH, CO2, DPG
Child with pneumonia and marked hypoxemia. Dx?
PCP. Dx with BAL
2 yo with recurrent hemoptysis, cough, anemia, and diffuse alveolar hemorrhage on CXR. Dx?
Idiopathic pulmonary hemosiderosis. Dx with hemosiderin laden macrophages on BAL
