Characteristics of psychotherapy
Verbal, two-group, voluntary, one expert and one help seeker, expectation of help, purpose of elucidating characteristic patterns of living that are troublesome.
What are the non-specific curative factors of psychotherapy?
“Placebo” factors, you want to maximize your placebo effect by intensifying the confiding relationship, expectation of help, abreaction, providing new info, providing a rationale for symptoms and having successful experiences.
What are the specific curative factors of psychotherapy?
Understanding, learning new tools, pattern recognition, learning from past experiences and practice.
What are essential tools you need to have for psychotherapy?
Inquiry (how to ask questions w/o offending), assessing, listening (figure out how the patient is wired), identify focal conflict (triggers, reminders).
5 types of psychotherapy
Psychoanalysis, psychodynamic psychotherapy, supportive psychotherapy, cognitive behavioral psychotherapy, interpersonal psychotherapy
A patient comes to see the psychiatrist because he always gets mad at men in authority over him. You analyze childhood neurosis and help him realize that it is because of experiences with his dad and he eventually leaves it behind him. What kind of psychotherapy is this? What other factor of his past did you try to get at during this therapy?
Psychoanalysis. Childhood neurosis and transference neurosis (repeated experiences with others) are targets in this type of therapy.
The most important predictor of successful outcome across all types of psychotherapy?
Therapeutic alliance: reality-based relationship with the patient, outside of the patient’s fantasies.
Your experiences of your past that influence your interactions with the patient.
Countertransference.
Types of countertransferance
Complementary (empathizes with someone in the patient’s life). Concordant (empathizes with patient).
A patient comes to the psychiatrist because of increasing intensity of anxiety since getting promoted. You focally analyze experiences with his father and with third grade leadership. You start him on medications. What kind of psychotherapy is this and what characterizes this type of therapy?
Psychodynamic psychotherapy. This type is more focal, analyzes defense mechanisms and is focused on the present.
A patient comes to see you in the psych clinic. She cannot remember the details of a traumatic experience. What type of defense mechanism is this?
Repression (involuntary withholding of feelings)
A patient with cancer refuses to say goodbye to people that come to visit her. What type of defense mechanism is this?
Denial
A mother yells at her child because her husband yelled at her. What type of defense mechanism is this?
Displacement (avoided ideas are transferred to neutral person)
You were a witness of a murder and describe the details with no emotional response. What type of defense mechanism is this?
Isolation of affect (separating feelings from events)
A child starts wetting the bed after their mom is hospitalized. What type of defense mechanism is this?
Regression (maturational clock is turned back to deal with the world)
