Treatment of acute prostatitis is
bactrim or floroquinolones for 6 weeks

fluoroquinolones
symptoms of chronic prostatitis
dysuria and increased urinary frequency
no symptoms of acute prostatitis (fever, chills malaise, and myaglia, abdominal pain or cloudy urine)
Recurrent UTI
diagnosis of chronic prostatitis
pyuria with possible tender prostate
urine culture positive usually E coli
causes of acute prostatitis
remote infection elsewhere, prolonged catherization, recent prostate surgery
Prostate massage is not recommended
in acute prostatis as this can cause septic shock
Digital exam in acute prostatitis
tender prostate that is warm and edematous
Treatment of acute prostatis if they appear septic
need IV antibiotics and the noral antibodies for 4 weeks and tailored to urein culture.
Afterwards need a urological evaluation to determine the underlying cause
chronic bacterial prostatitis is caused by
complication of ABP due to inadequate duration of treatment
how long to treat chronic bacterial prostatitis ?
may need a prolonged course of about 6 weeks to 12 months in some cases
urologists may obtain urine or prostate fluid specimens after prostate massage for diagnosis.

acute urinary retention
bladder drainage must be done through suprapubic catherization
Catheter insertion through an inflamed urethra into bladder is a contraindication.
when to order a transrectal U/S
to look for suspected prostatic abscess or cancer.
Prostate abscess can present similarly to prostatitis but has significant fluctuance on digital exam and persistent symptoms despite antibiotics.
how long is treatment for acute bacterial prostatitis?
6 weeks of ciprofloxacicin or bactrim because of excellent tissue penetration.
