Endemic mycoses (Blasto, histo, coccidio) Flashcards
disseminated blastomycosis is
from blastomyces dermatitidis and from inhaled conidia from environment that gets deposited into alveoli. May see SIRS with malaise, fever, cough and dypsnea.
what is on CXR with disseminated blastomycosis
see infiltrates that could be misdiagnosed as bacterial pneumonia
Presentation of disseminated blastomycosis
asymptomatic to ARDS
see cutaneous plaques and ulcerations,
bone lesions with sinus tracts
GU involvement
CNS involvement (is rare)
Which areas are endemic to Blastomycosis?
Mississippi / Ohio river valley
midwestern US
histoplasmosis is endemic to where?
Mississippi / Ohio river valley and Central and S. America
Which areas are endemic to Coccioidomycosis?
South Arizona, California and northern Mexico
presentation of disseminated histoplasmosis
hilar/mediastinal LAD
hepatosplenomegaly
pancytopenia
adrenal insufficiency
coccidioidiomycosis presentation
skin lesions, LAD, meningitis and osteoarticular infection.
Treatment of disseminated blastomycosis is
itraconazole as initial recommended treatment
Amphotercin B is for severe infection (like meningitis)
buzz word: What are the characteristic findings of blastomycosis on histology?
broad based budding yeast
actinomycosis
fungus that affects the pulmonary - extends across fissures and invades the chest wall and forms sinus tracts. IT can involve the bone or the cervicofacial area
what do we see with disseminated histoplasmosis?
see reticuloendothelial system involvement
also see hepatosplenomgealy and pancytopenia.
Mnemonic for BLastomycosis infection
Blasto Blasts Bones, Balls (epididymo orchitis) and skin.
what is associated with blastomycosis infection?
see a systemic granulomatous infection that involves the lungs causing a chronic pneumonia
See skin involvement with verrucous lesions, subcutaneous nodules, and bone (osteomyelitis) and genitourinary tract (prostatitis, epididymo-orchitis)
