invasive pulmonary aspergillosis risk factors
seen in severe immunocompromised state due to prolonged neutropenia, chronic high dose steroid use, solid organ and allogenic stem cell transplant or AIDS
classic triad of invasive pulmonary aspergillosis
cough, pleuritic chest pain and hemoptysis
diagnosis of invasive pulmonary aspergillosis
CT chest with nodules and ground glass opacity (halo sign) or cavitations with air fluid levels
serum biomarkers: galactomannan, beta D glucan (70% specific) but can have false positives with blastomyces and histoplasma
sputum sampling for fungal stain and culture
treatment of invasive pulmonary aspergillosis
voriconazole, reduction of immunosuppressive regimen
surgery if needed
