Gram positive rods in blood cultures differential diagnosis
corynebacterium species (diptheroid)
clostridium species
nocardia species or anerobic actinomyces species
listeria species
listeria monocytogenes is a
gram positive rod that is found in soil and decaying vegetables and often is a low level contaminant of food (fruit, unpasteurized milk, soft cheeses)
people who ingest large amounts of listeria are at risk for
self limited <48hrs febrile gastroenteritis and those with advanced age or underlying immunosuppression can also develop invasive disease (bacteremia, sepsis, pneumonia and meningitis)
listeriosis is confirmed by
blood CSF and cultures
do we treat listeriosis gastroenteritis?
No because self limiting.
cefazolin better for MSSA than vancomycin because
it’s more bactericidal.
