What is polymyositis and dermatomyositis?
AI disorders where there is inflammation in the muscles (myositis)
What is polymyositis?
Chronic inflammation of muscles
What is dermatomyositis?
Connective tissue disorder where there is chronic inflammation of skin & muscles
What can cause these conditions?
Can be caused by underlying malignancy
What does being caused by underlying malignancy make these disorders?
Paraneoplastic syndromes
Most common associated cancers with these diseases?
Lung
Ovarian
Breast
Gastric
Presentation of dermatomyositis and polymyositis?
- Muscle pain, fatigue & weakness
- Bilaterally & typically & proximal muscles
- Mostly affects shoulder & pelvic girdle
- Develops over weeks
Which one of these conditions has skin features?
Dermatomyositis
What are the skin features of dermatomyositis?
- Gottron lesions
- Photosensitive
- Purple rash on face/eyelids
- Periorbital oedema
- SC Calcinosis
What are Gottron lesions?
Scaly erythematosus patches
-on knuckles, elbows and knees
Auto-antibodies associated with polymyositis?
Anti-Jo-1 ABs (often also present in dermatomyositis)
Auto-antibodies associated with dermatomyositis?
- Anti-Mi-2 ABs
- Anti-nuclear ABs
What is the diagnosis of these diseases based on?
Clinical presentation Elevated CK Auto-ABs Muscle biopsy Electromyography (EMG)
Additional management of polymyositis and dermatomyositis?
- Assess for possible underlying cancer
- May require physio
Medical management of the myositises?
Corticosteroids = 1st line
Other options:
- Immunosuppressants
- IV Immunoglobulins
- Biological Therapy
