What is osteomalacia known as in children?
Rickets
What is Vitamin D?
Hormone created with cholesterol by skin response to UV radiation
What do kidneys metabolise vitamin D to?
Its active form
Presnetation of osteomalacia?
Fatigue Pain Muscle weakness Muscle aches Pathological fractures
Risk factors of osteomalacia?
- Darker skin
- Low exposure to sunlight
- Live in a colder place and spend a lot of time indoors
Investigation for osteomalacia?
Serum 25-Hydroxyvitamin D
If there is less 25nmol/L of serum 25-Hydroxyvitamin D?
Vitamin D deficiency
If there is 25-50nmol/L of serum 25-Hydroxyvitamin D?
Vit D insufficiency
Optimal levels of 25-Hydroxyvitamin D?
75nmol/L or over
Other findings in osteomalacia?
- Serum calcium is low
- Serum phosphate is low
- Serum alkaline phosphatase may be high
- PTH may be high
- X-rays may show osteopenia
Why would PTH be high?
Secondary hyperparathyroidism
Treatment of osteomalacia?
Vitamin D supplements
Colecalciferol
